Oral Health Program


The Oral Health program provides financial aid to children in low-income situations who require dental treatment for urgent or significant dental issues. This program is able to help eligible families with up to a maximum of $2000, once every 3 years.

The funding does not cover the cost of initial exams or regular check-ups. Families do not need a referral, but a treatment plan provided by a dentist or dental clinic is required.

Who Is The Program For?

  • Children who have urgent and/or significant dental issues and living in low-income situations
  • Under 21 years of age
  • Residents of Calgary for at least 6 months

Application Download Instructions:

1) To complete an application on your computer, please right-click on the form link and select “Save Link As…”
2) Save the file to your desktop or a folder.
3) Open a program like Adobe Reader to fill out and save the form.

An application to the Oral Health Program is attached below. This application will be used to determine if children qualify for assistance. It is important to attach everything that is asked for, where applicable, or the application process will be delayed. If something is missing, please explain why.

Supporting Documents Required:

  1. INCOME DOCUMENTS – two months of recent paystubs, employment insurance, Alberta Works income support, student loan, AISH, etc.
  2. ESTIMATE OF DENTAL COST AND TREATMENT – families can get from the dental clinic.
  3. PROOF OF ADDRESS – rent or mortgage agreement, utility bills, etc. [HSB example]
  4. CANADA CHILD BENEFIT (CCB) statement
  5. I.D. FOR CHILDREN – Alberta health care card(s) for children, passport, birth certificate, etc.
